How to send in documents
Uploading documents electronically
Scan and save your documents, or use a smart phone to photograph the document and upload it to your computer. A library or community agency can help you.
Log into HealthCare.gov with your account information.
Click on the “Continue” button
Click on your most recent application.
Click on “Application details”
Click “Upload Documents”
Under “Document Type” click on the type of document you want to send, and click on the “Select File to Upload” button.
Pick the document you scanned/photographed in step 1, and click “Upload”
Sending documents in the mail
Mailing documents takes much longer and they could get lose in the mail. Make sure you give enough time for the documents to arrive if your documents are needed by a certain date. If you have a page with a bar code on it from your Eligibility Determination, put this on top when you mail your documents.

Write this information on every page you send in:
Name
State of residence
Application ID number
How to report life changes:
Life events and changes in income need to be reported to HealthCare.gov. Some qualifying events include:
Moving
Becoming a US citizen, resident, or national
Turning 26 and losing your parent’s insurance
Had a baby or adopted
Marriage or divorce
You have new tax dependent
Death of an immediate family member
You’re an American Indian or Alaskan Native
Reporting changes on the phone
To report a change over the phone, call 1-800-318-2596. If you make more income than you reported, you may have to pay back some or all of the financial help you receive.
Reporting changes online
Log into HealthCare.gov
Click on “Continue”
Click on your most recent application
Click on “Report a life change”
Click on the type of life change you want to report, and click “Continue”
Follow the on-screen instructions to finish your reporting.
